1.  I believe that's Mark McLemore of Seattle and Casey Candaele of Cleveland.

2.  I retract that it's Casey Candaele. He never played for Cleveland at home against Seattle.

3.  That kind of looks like David Segui, but it's hard to see the back of his uniform. Is that #26?

4.  I'm guessing the "5" on the sleeve is in honor of Lou Boudreau, who died 8/10/2001.

Unfortunately, #26 in 2001 was Bob Wickman, and that sure as heck ain't Bob Wickman.

5.  I'm going with October 14, 2001 AL Division Series

Marty Cordova is the Cleveland player.

Third inning of game 4:

INDIANS 3RD: Cordova singled to center; On a bunt Diaz forced
Cordova (third to shortstop);

6.  I'm pretty sure it's 26, too. In '99, 26 was Jesse Levis, and in '98 it was Jeff Manto (who also wore #12 at some point).

8.  5 Okay, I see that it could be 46, too.

10.  I think Cordova and 2001 is right.
The patch on McLemore's sleeve would be this:

11.  I think Bob nailed it in 5. Long sleeves back up the October date, everything else (Cordova, #46, McLemore's patch, Boudreau's #5) matches.
